Job Responsibilities
- Ensure deliverables of site-wise activities and NCD screening targets are achieved.
- Coordinate with ART Centers for NCD screening and link clients to required services.
- Roll out NCD self-care literacy materials and support patient literacy sessions.
- Support data recording, tracking and monthly analysis; ensure data confidentiality and compliance.

Key Responsibilities (summary)
- Design and develop monitoring & evaluation plans, formats and reporting systems for partners.
- Validate partner reports, ensure data quality and perform MIS data analysis.
- Identify training needs of SSR teams and provide on-site capacity building.
- Support assessments, evaluations, and produce monthly/quarterly reports.
- Liaise with SACS and other stakeholders for coordination and data sharing.

GFATM SAMAGRA Project in Sangli District.

Core Responsibilities of Selected SSR:
- Operate the Care & Support Center (CSC) as per GFATM guidelines.
- Provide treatment adherence, psychosocial support & referral services.
- Ensure data entry, M&E compliance and timely reporting.
- Manage district-level PLHIV engagement and outreach.
